8. Fill the Patient Details Form

1. Enter your Email Id

2. Enter the Patient Name and Age

3. Select the Date Of Birth

4. Enter the Patient Phone Number

5. Select the Gender

6. Click on the button labelled "SELECT PATIENT"

9. Click on SELECT ADDRESS

1. Enter your Address

2. Enter your City Name

3. Enter your State name

4. Enter your Area pin code

5. Click on the button labelled "SELECT ADDRESS"

10. Click on Review Order

1. Select the Appointment Date & Time

2. Click on the button labelled "Review Order"

11. Click on CHECKOUT

Click on the button labelled "CHECKOUT"

12. Appointment Confirmation

1. Select the payment method and do payment

2. After payment you can view the confirmation message on the screen

3. You will receive a confirmation call from the Rays Diagnostics team
