Navigate to http://supratechlabs.com/

2. Click on the Page

3. Click to select Location

1. Click on the Location Icon

2. Select the Country

3. Select the City

4. Click on Book a Home Visit

1. Scroll down to view this options

2. Click on the button labelled "Book a Home Visit"

5. Enter your test in the Search box

1. Scroll down to view this Package/Test Details Form

2. Search for the test using the search box

6. Click on Ok

1. Click on the test name once you find it

2. Your test will be added to Cart

3. Click on the button labelled "Ok"

7. Click on Finish Booking

1. Enter patient First name and Last name

2. Enter your Email Id

3. Select Gender

4. Enter your Mobile number

5. Enter your Date Of Birth

6. Enter your Address with landmark and pin code

7. Select the Appointment Date & Time

8. Select the Payment Option - Cash on Service

9. Click on the button labelled "Finish Booking"
