3. Click on GET ONLINE PATIENT REPORT
Scroll down the home page to see "GET ONLINE PATIENT REPORT" and click on it.
4. Enter your Username and Password
a. Click on the Group Name and select patient
b. Enter your Username and Password on the page
c. Enter your Username as Password as printed on the payment receipt
5. Click on Login
a. Click on the button labelled "Login"
b. You can view the list of test reports
c. Select the test and download the report
