2. Click on button for Download Reports

Click on the button labelled "Download Reports"

3. Select the City

1. Select the city where you have registered

2. Click on the button labelled "Submit"

3. Click here to visit the report page.

4. Enter your User ID and Password

1. Enter the User ID and Password on the page.

2. Your User ID and Password will be on your payment receipt.

5. Select "Login as" Patient

Click on the "Login As" and select patient.

6. Click on Login Button

Click on Login Button and wait for the next page to load.

7. Click the Download button

1. You can view your reports on the page

2. Click the first button situated on the top right of the download bar. This will save the report to your device.
